Required Forms

Commitment Form - this form, to be signed by both the student and a parent, acknowledges that you have read the Band Handbook and that you understand its contents.

Health and Permission Form - This form gives the student permission to go on recurring trips with the band and also provides general information about medications.

Media and Video Release Form - This form gives the band permission to use pictures and videos of the student on the website and social media platforms.

Instrument Rental Form - This form is to be filled out by all students needing to use an instrument owned by the Union County Bands.

General Forms

Absence Request Form - This form should be completed and turned in a week in advance of any anticipated absences.

Parent Profile Form - This form contains information about ways in which you as a parent can get involved and help the band be successful in our goals.
